MANAGED CARE & FINANCIAL INFORMATION
The surgeons and staff of Capital Orthopaedic & Sports Medicine Center are
committed to providing the finest in medical and surgical care.  We are willing to
discuss our professional fees and policies at any time.  A detailed description of
our financial policies is available for your review.  Policies include the following:

 * Self-pay:  Self-pay patients without any insurance coverage are expected to
pay in full at the time of each visit.  Medicaid application may be possible,
depending on the circumstances.

 * Insurance:  We are pleased to help patients with health insurance to receive
the full benefits to which they are entitled.  We routinely file insurance for our
patients when the proper forms and information are provided.  Professional
services are charged to the patient and not to the insurance company;
therefore, the patient is responsible for timely payment of the account.  
Deductibles and co-payments are expected to be paid at the time of service.  
When insurance companies fail to pay an account within 45 days, the patient is
then responsible for payment of unpaid charges within 15 days.  Late payment
charges (finance charges) are added to unpaid accounts after 120 days from the
date of service.
 * HMOs:  We accept assignments from several managed care companies.  Ask
us about our specific coverage.  Patients are responsible for co-payments at the
time of service.

 * Blue Cross/Blue Shield:  We are a COSTWISE provider and accept
assignments for BCBS.  However, the patient is responsible for payment of the
deductible and co-payments.

 * Worker's Compensation:  All worker's compensation claims must be verified
and approved in writing by the employer.  Patients seen by another physician for
the same complaint must present an authorization for the change of physician
that is verified on the employer's letterhead.

 * Personal Injury With Attorney:  The patient, not the attorney or another third
party, is responsible for treatment charges.  Patients must sign our lien form to
authorize their attorney to pay us directly for any balance remaining for
professional services rendered.
